1. Introduction

These Terms of Use govern your use of this website and the services of Healthbase. By using the site, requesting a call, or working with one of our agents, you accept them. If you do not accept them, please do not use the site.

2. Who we are

Healthbase is a licensed insurance agency at 30 N Gould St, Sheridan, Wyoming 82801.

Insurance licensing is granted state by state. We are licensed to transact insurance business only in the states where we hold a current licence, and nothing on this site is an offer to transact business in any other state. Ask us and we will confirm whether we are licensed in yours before anything proceeds.

3. What we are not

Healthbase is not a government agency and is not connected with or endorsed by Medicare, the Centers for Medicare & Medicaid Services, the Health Insurance Marketplace, or any federal or state program. Nothing on this site is an official government communication.

This website is a solicitation for insurance. We are an agency, we sell insurance products, and we are compensated by carriers when clients enrol through us. A licensed agent may contact you if you ask us to.

We do not represent every plan available in your area. Any information we provide is limited to the plans we do offer. For the complete picture, use theMedicare Plan Finder, call 1-800-MEDICARE, contact a freeSHIP counsellor, or useHealthCare.gov.

4. Scope of what we do

We explain how the three systems work, compare the plans we are appointed with against your doctors and prescriptions, submit an application when you ask us to, and help afterwards when something goes wrong.

Everything on this site is general information, not advice about your situation.Rules, plan availability, networks and pricing change annually and vary by county and household. Nothing here is a quote, and nothing here is an offer of coverage. Only the documents issued by a carrier state what is actually covered.

We deliberately publish no premiums, deductibles, copays, percentages or income thresholds anywhere on this site. Those figures change and a dated figure on a licensed agency's website is worse than no figure at all.

Nothing on this site enrols you. Submitting a form is a request that we contact you. It is not an application, and it does not hold a place, reserve a plan or protect an enrollment deadline. If a deadline matters, telephone (866) 346-1635 rather than relying on a web form.

5. How we are paid

Carriers pay us a commission when you enrol. Our help costs you nothing, and your premium is identical whether you enrol through us, through another agent, or directly with the carrier — premiums are filed with regulators and carry no agent mark-up.

The honest implication is that an incentive exists, which is why a whole page of this site is about it.

6. Your part

You confirm that you are at least 18, that you have authority to arrange coverage for yourself or for the person you are enquiring about, and that what you tell us is accurate to the best of your knowledge. Inaccurate information about doctors, prescriptions or existing coverage produces a recommendation that does not fit.

Please also do not:

• Send a Social Security number, Medicare number or bank details through any form on this site.
• Submit another person's details without their authority.
• Attempt to disrupt the site, scrape it wholesale, or reach parts of it you are not meant to.

The text, layout and design here belong to us. Read, print and share pages freely; do not republish them as your own.

7. Limits and liability

The site is provided as it is. To the fullest extent permitted by law we disclaim warranties of accuracy, fitness and availability, and we are not liable for losses arising from relying on general information published here rather than on advice given to you directly or on your plan documents.

This limitation concerns the website. It does not limit liability arising from the agency services we provide to clients, and it does not exclude any liability that cannot be excluded at law, including for fraud or for breach of the duties state law imposes on licensed producers.

We link to Medicare.gov, HealthCare.gov, SHIP and carrier portals because they are useful and free. We do not control them and are not responsible for their content.
